**IMPORTANT** Veterinary and Owner Consent:

Veterinary consent is a legal requirement when booking an initial appointment. Owner consent is also required as part of our Terms and Conditions. I will need to receive both prior to your appointment. Contact us to receive the relevant forms or, download them below. On the Veterinary consent form fill out the owner and pet details and sign. The form can then be emailed to your vet for them to sign and provide consent for treatment. The Owner’s consent form is for the animal’s owner or responsible person to fill out. Once completed, please return to raybrookvp@outlook.com
